Title 극소 저체중 출생아에서 동맥관 개존의 치료 방침에 따른 동맥관과 관련된 동반 이환율 및 사망률 및 장기적 신경학적 예후

Background 혈역학적으로 의미 있는 동맥관 개존에 대한 치료 방법에 대해서는 아직도 많은 논란이 있다. 과거 국내에서는 특히 초미숙아에서 보존적 치료의 빈도가 높고 사망률이 높은 것으로 발표된바 있다.
Aim / Hypothesis 지난 10년간의 동맥관 개존에 대한 치료 방침의 변화에 따른 임상 경과 및 이환률과 사망률 그리고 생존 환아의 장기적 신경학적 예후의 변화를 알 보고자 한다
Inclusion Criteria 2013.1.1.-2023.12.31.(최근)까지 출생하여 Korean Neonatal Network database (KNN)에 등록된 extremely preterm infant및 극소 저체중 출생아 (출생 체중 1,500 그램 미만의 신생아)
Exclusion Criteria (1) 주요 선천성 기형 (2) 동맥관 개존의 증상이 없고, 치료도 받지 않은 환아 (3) 생후 3일 미만에 사망한 환아
Study Design Statistical methods Population-based retrospective cohort study 통계적 방법 One-way ANOVA or Kruskal-Wallis test Chi-square test with multiple comparison Multivariable logistic regression
Primary Outcomes Mortality >2 postnatal days, within 1months Composite outcome : mortality > 2 postnatal day, Intraventricular hemorrhage (IVH) ≥ grade3, Periventricular leukomalacia (PVL), Necrotizing enterocolitis (NEC) ≥ stage 2, moderate to severe BPD, Retinopathy of prematurity (ROP) requiring surgery or VEGF
Secondary Outcomes and Definitions Each major morbidity : Intraventricular hemorrhage (IVH) ≥ grade3, Periventricular leukomalacia (PVL), Necrotizing enterocolitis (NEC) ≥ stage 2, BPD, severe BPD, BPD or death, severe BPD or death, Retinopathy of prematurity (ROP) requiring surgery or VEGF, ROP or death Long-term outcome : Motor delay, Cognitive delay, Language delay, Cerebral palsy, Hemiplegia
Protocols 환자군의 분류 (1) No symptomatic PDA (2) Prophylactic treatment of PDA (3) Pre-symptomatic treatment of PDA (4) Symptomatic treatment of PDA (5) Conservative treatment of PDA without any pharmacological, surgical intervention In-hospital outcome의 비교 [입원 기간, 호흡기 치료 기간 [invasive ventilator care, non-invasive ventilator care, supplemental oxygen, 퇴원시 산소 또는 home ventilator 유무 포함] 및 산전 및 치료 전 교란변수를 보정한 mortality 및 major morbidity 비교 장기 outcome 교정나이 18-24개월 및 3세 추적 항목 : 특히 발달 관련 정보로 Motor delay, Cognitive delay, Language delay, Cerebral palsy, Hemiplegia
