Title Long-term outcomes of very low birth weight infants with congenital anomalies during the first 3 years: a Korean neonatal network

Background Management of newborn infants with congenital anomalies (CA) is challenging and requires a multidisciplinary approach. CA have been thought to be a risk factor for mortality, growth failure, and neurodevelopmental impairment in preterm infants. However, the impact of the type of CA on the mortality, growth, and neurodevelopment remains unknown.
Aim / Hypothesis To investigate the association of the type of CA and mortality, growth and neurodevelopment in VLBWIs registered in Korean Neonatal Network during the first 3 years. 1) The group with congenital anomalies will have a higher mortality, growth failure, and neurodevelopmental impairment during the first 3 years than the control group. 2) Among sub-groups of CA, the survivors with congenital heart defects will have increased the risk of growth failure, and those with central nervous system and chromosomal anomalies, will have increased the risk of neurodevelopmental impairment.
Inclusion Criteria All VLBWIs registered in KNN from 2013 Jan to 2017 Dec
Exclusion Criteria 1) Missing data of parameter of “major congenital anomaly” 2) Congenital infection
Study Design Statistical methods Case - control design 1:3 matching for same sex, multiplicity and gestational age Risk analysis: chi-square, t-test, hazard regression for survival
Primary Outcomes Mortality for 3 years 1) single versus multiple 2) type (organ system)-specific analysis
Secondary Outcomes and Definitions Weight/height/head circumference at discharge, 18-24 months of corrected age, and 3 years old. Bayley scales test, K-DST at 18-24 months of corrected age, and 3 years old.
Protocols 1) Eligibility criteria and demographics 2) 임신, 분만, 신생아 정보 3) 질환정보 I – 공기누출증후군, RDS, 폐표면활성제 사용여부, BPD예방을 위한 스테로이드사용 여부, 교정연령 36주 시점 및 기관지폐이형성증, 4) 질환정보 II – PDA 약물치료, 동맥관결찰술: 뇌실내출혈 단계, 뇌실주위 백질연화증, 패혈증 여부 있음/없음/미시행 5) 질환정보 III – 정맥영양기간, 괴사성장염 stage2 이상, 최초진단일, 괴사성 장염으로 인한 수술 여부, 미숙아 망막증 병기, 미숙아망막증 수술, 심각한 선천성 기형 여부, 선천성기형종류 6) 퇴원 관련 정보 – 퇴원일/입원일/재원일수, 퇴원형태 (가정/전원/사망/365이상입원중), 퇴원시 체중/신장/두위, 사망원인 7) 방문 및 치료 정보(FU1), 발달 정보(FU1) 8) 방문 및 치료 정보(FU2), 발달 정보(FU2)
