Title Risk factors for air leak syndrome and outcomes in very low birth weight infants

Background Pneumothorax는 full term infants에 비해 preterm infants에서 incidence가 더 높으며, 발생률은 VLBWI에서 2.3%, ELGANs에서는 10%까지도 보고되었다. 그리고 lung protective ventilatory strategy, early selective surfactant가 pneumothorax를 감소시키고, delivery room에서 양압환기가 pneumothorax의 빈도를 증가시킨다. Pneumothorax가 동반된 경우 만삭아에서는 mortality, IVH등의 위험이 증가하지 않지만 GA 28주 미만 미숙아에서는 그 위험이 증가하였다. ELGAN에서 surfactant strategy나 초기 소생술에 따른 pneumothorax의 연구와, IVH외에 pneumothorax가 관련있는 합병증이 있는지, 후기 발달에 영향을 미치는지에 연구가 더 필요하다.
Aim / Hypothesis ELGANs에서 air leak syndrome (pneumothorax)의 위험도를 증가시키는 인자와 ALS를 동반한 아기들에서 mortality, morbidity를 알아본다. GA에 따른 ALS 의 incidence와 complication의 추이를 알아본다.
Inclusion Criteria 2013-2020년 출생하여 KNN에 등록된 VLBWI 중 GA 28주 미만 출생한 신생아
Exclusion Criteria major congenital anomaly no available data about air leak
Study Design Statistical methods 1) 2013-2020년 출생하여 KNN에 등록된 아기들 중 GA 27주 미만의 아기들에서 ALS의 incidence를 조사, GA에 따른 ALS의 발생률을 알아본다. 가능하면 ROC curve를 통해 ALS가 증가하는 cut-off GA를 구한다. 2) ALS 발생한 아기들과 GA, birth weight에 대해 propensity score matching하여 non-ALS group을 정한다. 3) ALS 군과 non-ALS군 사이에 risk factors를 chi-square test, Fisher's exact test, Student t-test, Mann-Whitney test를 통해 비교한다. 4) Multivariable logistic regression을 시행하여 ALS의 위험도(OR)를 높이는 risk factors를 찾는다. 5) ALS 군에서 GA에 따른 mortality, IVH, BPD, mortality + IVH, mortality +BPD의 유병률의 변화를 알아본다. 가능하면 ROC curve를 통해 mortality 및 morbidity가 증가하는 cut-off GA를 구한다. 6) ALS 군과 non-ALS군 사이에 morbidity, mortality를 chi-square test, Fisher’s exact test를 통해 비교한다. 7) Multivariable logistic regression을 시행하여 ALS가 mortality 또는 morbidity의 위험도(OR)를 높이는지 알아본다. 8) ALS군과 non-ALS군의 퇴원 후 BSID-II, K-DST를 비교한다.
